Leicestershire County Council, in collaboration with Leicester City Council, is currently in the planning stage for recommissioning the "Hospital to Home Service." This service aims to facilitate the safe return of individuals from hospital to their homes, focusing on Assisted Discharge and Home from Hospital support.
