The Council aims to ensure improve Service Users remain as independent as possible; have the knowledge to make informed decisions; have choice and be in control of decisions made about their Care; be part of their community; receive Support at the right time through a strength based and personalised Service. The provider delivering the Service, will aim to achieve the following objectives: enhance the quality of life for Service Users with Care and Support needs; delay and reduce the need for Care and Support; ensure Service Users have a positive experience of Care; safeguard Service Users whose circumstances make them vulnerable and protect them from avoidable harm; improve and maintain Service Users health and wellbeing through increased independence, choice, control, dignity, and quality of life; promote independence, and enable the Service User to be an active member of society and their community; provide a strength based support Service, responsive to the needs and circumstances of the Service Users; help maximise independence after illness or disability through working within a rehabilitation, reablement and enablement philosophy; provide a high quality Service in terms of access, the Service Users' experience and outcomes based on evidence of effectiveness; enable Service User satisfaction, enabling Service Users to have a say in how Support and Services could be developed in the future, ensuring meaningful engagement that enables Service Users to have a say about the future development of Services. provide clear information so that Service Users can make informed choices. The contract will run from 01/4/2025 to 31/03/2028 with the option to extend by a year to 31/03/2029.
