NOTIFICATION OF INTENTION TO AWARD: The Life After Stroke Service will provide a seamless experience for stroke survivors to provide a person centred and needs based service that supports stroke survivors whilst reducing inequity of provision across Sussex and preventing secondary strokes. This service will deliver the following key elements of the NHS recommended integrated Life After Stroke service model. These are the core elements based on stroke survivor and carer/family priorities identified through clinical and public engagement. The core elements are: * Post Stoke Reviews * Supported Self-Management Education * Self-Management Peer Support Contract length: 5-year contract with an option to extend for a further 2-years Annual contract value: PS372,153 Total contract value (5-year + 2-year option) = PS2,605,071 Contract service dates = 2nd April 2025 to 31st March 2030 2-year extension dates = 1st April 2030 to 31st March 2032
