Lot 2 will be placements in Care Homes individuals who may: * require care and support over a twenty-four (24) hour period, including observation and/or supervision to maintain safety; * have a high level of assessed care needs; * require assistance with most activities of daily living; * require input by care workers on a daily basis; and * require Nursing Care provided by community services.
Lot 6 will be placements in Care Homes with Nursing Care (such terms are defined in the Specification) for individuals who may: * require care and support over a twenty-four (24) hour period, including observation and/or supervision to maintain safety; * have a high level of assessed care needs; * are in receipt of NHS Funded Nursing Care (FNC); * require assistance with most activities of daily living to maintain skills and independence; * require input by care workers on a daily basis; and * require Nursing Care on a daily basis.
Lot 7 will be placements in Care Homes with Nursing Care (such terms are defined in the Specification) for individuals who may: * require care and support over a twenty-four (24) hour period, including observation and/or supervision to maintain safety; * have a high level of assessed care needs; * are in receipt of NHS Funded Nursing Care (FNC); * require assistance with most activities of daily living to maintain skills and independence; * require input by care workers on a daily basis; and * require Nursing Care on a daily basis.
