These beds are for short term placements (maximum stay of up to 6 weeks) to support individuals who require an enhanced level of care following discharge from an acute hospital or a community hospital rehabilitation setting or to avoid an admission to hospital are for patients with dementia and who have enhanced level of need due to challenging behaviours. Individuals who do not (or no longer) require acute care or clinically led rehabilitation care, but whose level of need and complexity precludes safe delivery of care at home, can be admitted to a short-term intermediate care placement. Effective date of the modification 1st October 2025 There is no change to the duration of the contract, which remains 1 April 2022 to 31 March 2030, inclusive of a 3 year optional extension.
