The Landlord will be the Local Authority and the Young Person's occupancy will be under a licence to occupy. The Service Provider will focus on delivering a highly bespoke trauma informed care and support, working as part of a multi-disciplinary group. The service will be person centred, utilising structured proactive approaches to help the Young Person reach his full potential and smooth transition into adulthood. Tender Option 2 seeks proposals from experienced and suitable organisations to provide a solo residential care placement to be registered with the Care Inspectorate for Wales. The Provider is required to source accommodation within the Conwy area and deliver a tailored, outcome focused service in line with the Young Person's needs to help him reach his goals and full potential. The Service required will be built on adopting proactive and therapeutic approaches in relation to the Young Person's behaviour, working closely with other agencies e.g. BCUHB. As he reaches adulthood, intentions are to maintain the continuity of the Provider's service under a less intense environment and provide support that enables a smooth transition into independent living.. The contract is required with immediate effect with timescales agreed in line with the Provider's tender proposal.
