The project will be delivered by an experienced arts organisation/s with a track record of delivering high quality arts provision to improve health and wellbeing. The expected outcomes are as follows: Improve wellbeing Enhance health outcomes - reduce stress, improve mental wellbeing, and increase Self-management of chronic conditions Reduce demand on clinical services Increase social inclusion Foster patient activation - providing skills, information and confidence in living well with illness
