Lot 4 Children with Disabilities: We are seeking Provider to offer specialism, included one or more of the following support needs: Physical Disability, Sensory loss, Learning Disability, Autism Spectrum, Attention Deficit Hyperactivity, Emotional Behavioural supportWe are seeking providers to establish a block contract with us. Preferably located within Reading area. Ideally across more than one site to allow more effective management of matching placements. Services are required for children and young people with physical or mental impairment that has a substantial and long-term adverse effect on his/her ability to carry out day-to-day activities. Lot 5 Children's Home 16/17 year olds with care and support needsWe are seeking Provider to offer children's home service to 16/17 year old children with care needs, with aim to either support child into independence at 18 : or to work with Children's Services and Adult Social care to manage an appropriate move on to Adult Services provision when care needs meet Adult Services threshold. Lot 2 Therapeutic Residential Care We are seeking providers to establish a block contract with us. Demonstrating expertise to deliver therapeutic care, within an identified, evidence-based, nationally recognised, therapeutic model; for children with complex psychological, emotional and behavioural needs, including EBD and including sexualised behaviours. This service will deliver child centred service to support long term stable placements of children. The service will build a flexible approach around the needs of child in placement and work with Social Care to support with any changing needs. Block 2 bedsAge range 9-18
