Adults aged 16 and over who have a diagnosed learning disability and who require support with a variety of needs within their home and in the community. Adults aged 16 and over who have mental health and/or substance misuse issues and who require support with a variety of needs within their home and in the community. Mostly housing support, support and care but with some personal care requirements as necessary. This relates to Adult service users aged 16 - 64 who present with significant challenging behaviour and/or complex autism. This support requires well experienced staff who are appropriately trained and skilled in managing individuals with very complex needs within their home and the community. Adults aged 65 and over who have age related frailty and may have diagnosed mental health issues and who require support with a variety of needs within their home and in the community. Mostly personal care, support and care but with some housing support requirements as necessary. Adults aged 65 and over who have a physical disability or acquired brain injury and who require support with personal care and/or non-personal care within their home and in the community. Adults aged 65 and over who have age related frailty and may have diagnosed mental health issues and who require support with a variety of needs within their home and in the community.
