Supported Living & Outreach provision Framework

Framework for provision of Outreach, Support Living without accommodation and Support Living with accommodation. The services provide an important front-line service to some of our most vulnerable young people and adults with Learning Disabilities, Physical Disabilities, Autism Spectrum Disorder (ASD) those living with Mental Health conditions, people with complex needs or multiple conditions and people with a dual diagnosis of substance misuse/mental health conditions. Some of the service users that the Council supports have very complex and/or multiple support needs.

The opportunity has been divided into a number of lots and sub-lots which include: Lot 1: The Council invites Tenders for the provision of Outreach Services: Outreach services - is the provision of one to one support to service users to enable them to play an active part in their communities. The service will work to improve the physical, social and mental wellbeing of service users. Outreach support is tailored and linked to the individual and not to a specific building or location.

The support could end, or the Provider could change, and the person would not have to move. As such outreach support can be delivered across all tenures. Lot 2 & Lot 3: The Council invites Tenders for the provision of Supported Living Services .

The Services will be provided for people who live in a variety of settings as follows: i) Scheme Based (Supported Living) services including: Self-contained flats in a scheme or perhaps as a cluster of properties with onsite support and in some cases, a communal hub Shared houses with floating or onsite support - in these types of schemes communal facilities like kitchens and bathrooms might be shared Tenderers bidding for Lot 3 and all relevant sub-lots will be expected to demonstrate they are able to source suitable accommodation or work with other partners to be able to source suitable accommodation.
