Lincolnshire County Council (LCC) is initiating a procurement process, titled "Transitional Care Beds 2025," targeting the health and social work services sector. Interested Care Home providers must submit their tenders by 24th February 2025, via electronic submission. The procurement method is an open procedure, ensuring fair competition and transparency. This procurement seeks to enter into additional block purchasing agreements that will offer additional and fixed capacity for Older Persons transitional and nursing care. By securing such capacity the Purchaser will be in a stronger position to be able to manage increasing demand within existing financial and market-based constraints. Transitional and Reablement Beds will provide various levels of support to Older Persons who require a period of further assessment to determine their longer-term needs with the focus being on returning home. This will be achieved in partnership between Purchaser and the Provider. The service will provide early intervention, assessment, support and care planning for the Service User and will work with the individual and the Care Home staff to help achieve their identified goals and outcomes. The procurement will be carried out by Lincolnshire County Council (LCC) on behalf of Lincolnshire Community Health Services (LCHS). The requirement is for a block of fifty (50) Transitional Care beds, twenty seven (27) of which are Nursing beds with the remaining twenty three (23) being residential care beds. This cost reflects the complexity and multi-purpose usage of the beds. Costs will be fixed with the rate for nursing beds set at PS1000.88 per bed per week and transitional care beds rate per week set at PS764.00 per bed per week. This rate will apply upon contract commencement date until 31st March 2026. The geographical location of the beds must be within a 10 mile radius of the below hospitals: Lincoln County Hospital - LN2 5QY Pilgrim Hospital Boston - PE21 9QS Grantham and District Hospital - NG31 8DG County Hospital Louth - LN11 0EU
