Assertive Outreach Service

The Assertive Outreach service ensures a persistent and purposeful approach is used to engage with individuals who do not present to, or have difficulties engaging with housing, homelessness, or health and social care services. This approach is particularly effective for people who are isolated, alienated, unwell, or disabled, and who are unable or unwilling to attend services and keep appointments In the context of Assertive Outreach, 'assertive' means being proactive and persistent in efforts to engage with individuals. It involves: - Persistent efforts to locate and engage with individuals in their own space and at their own pace. - Providing persistent practical assistance and support to help individuals' access services and make informed choices. - Employing a non-judgmental and empathetic approach to build trust and rapport with individuals.
