BLMK ICB are intending to award a contract an existing provider following direct award process C for the following:<br/><br/>Community Gynaecology Service: This service provides assessment and treatment for a range of gynaecological conditions in a community setting, aiming to reduce secondary care referrals. The service includes diagnostics, minor procedures, and onward referrals where necessary. It also supports GP education and pathway development. <br/> <br/>Community Cardiology / Integrated Community Heart Failure Service: This dual service offers tiered cardiovascular care, including diagnostics, management plans, and follow-ups for non-acute cardiac conditions. It supports seamless transitions between primary and secondary care, and includes a sub-speciality for heart failure management. The heart failure component focuses on medication optimisation, patient education, and end-of-life planning, with strong MDT collaboration and integration with rehabilitation and hospice services. <br/> <br/>Integrated Community Respiratory Service: A specialist-led service for diagnosing and managing respiratory conditions such as COPD and asthma. It includes spirometry, home oxygen assessments, clinics, and ANP-led reviews. The service promotes early diagnosis, self-management, and integration with smoking cessation, mental health, and rehabilitation services.<br/><br/>The contract is for a period of 2 years from 1st April 2026. The expected lifetime value of the contract is PS2,937,019.
