The Community Occupational Therapy Service will deliver a high-quality outcome-focussed service supporting people to achieve their identified outcomes. The Community Occupational Therapy Service will: * Support local people live independent, safe and healthy lives in a place they call home where they feel safe and part of their community. * Help people to live fulfilling lives regardless of age, disability, status and social background * Facilitate and enable people to overcome any barriers that prevent them from doing the activities (occupations) that matter to them. * Support people to increase their independence * Support carers to maintain their caring role and achieve choice and control in their life outside of caring.
