This is a notice to inform the market of the intention to award a contract for Community Support and Social Recovery for the population of Leeds via the Most Suitable Provider Process.<br/>The Health Care Services (Provider Selection Regime) Regulations 2023, Paragraph 10 - The Page 4 to 5 Most Suitable Provider Process is being used to commission this service. This notice will be live for fourteen (14) days from the date of publication. This is a notice to inform the market of the intention to award a contract for Community Support and Social Recovery for the population of Leeds <br/>The aim of the service is to provide Community mental health support for enduring mental health needs; services working as part of the community mental health transformation programme, within the Leeds Health and Care Partnership.<br/><br/>There has been extensive work undertaken over the last 2 years, through service reviews, consultation and place committee approval to develop a joined-up approach to the delivery community mental health services in the city. Through a lead provider approach, it is expected to reduce the risk of miscommunication or gaps, and therefore improve patient management/safety. As part of the Leeds Community Mental Health Transformation programme, The aim of the service is to support people with complex and enduring mental health needs to live as well as possible in their communities, through community based services within the well-established Leeds Health and Care Partnership.<br/><br/>The Provider should be able to deliver and evidence through; Working closely with VCSE organisations; support for people to achieve better mental health and wellbeing, increasing independence and supporting recovery, focused and assertive outreach support for people with the most complex and enduring needs. It will also provide support to inpatients to prepare them for discharge, with the aim of helping people feel prepared for going back into the community when they are medically fit for discharge.<br/><br/>The estimated value of the contract is PS6,300,000 for a 3+2 year contract.
