a) Group provision- Offering a range of group activities for children and young people which includes opportunities for social inclusion and enrichment within the community with other children and young people. This may include increased provision during school holidays in line with the child's assessed needs. b) Outreach- Offering 1:1 (or staffing ration in line with assessed need) support to children and young people within the community (or home setting if required) to work towards outcomes identified within the support plan. Where shared outreach services (2:1 child: staff ratio, or other suitable ratios) would be beneficial to children and young people these may be agreed through Stockton on Tees Borough Council's panel process, subject to risk assessment and agreement with the council. Offering a range of group-based activities for children and young people within the school holidays. This could include a building's based activity or an activity within the community.
Offering personal care support to children and young people with an assessed need for personal care support
Offering opportunities to promote social and independence skills to meet outcomes as identified in their Transition care plan. Where possible young people will be engaged in education, where additional needs have been identified above their education support, day opportunities may be required.
Offering support to children and young people as identified within their assessment where a bespoke package of care is required, this may be short term to provide a period of support or longer term to prevent family breakdown.
