Delivery will also include the National Child Measurement Programme (NCMP), contributing to local and national public health intelligence.
This lot enhances the HCP through community outreach and supporting families from pre-birth to age two. The service will prioritise the six early years high impact areas, offering tailored support such as enhanced antenatal care, breastfeeding support, maternal mental health support, and health education. By integrating with existing services and co-locating within Family Hubs, the model aims to strengthen pathways and ensure equitable access to preventative and early help services.
