This service is for people aged 18 years or over who need support after discharge from an acute inpatient setting, virtual ward or to help someone in a crisis (step up service from community ), to help them rehabilitate, re-able and recover, before returning home to their usual place of residence.<br/><br/>Contract period: 1 April 2025 - 31 March 2030<br/>Contract Value: PS4,574,700
