Summary, notice description and lot information
NHS Humber & North Yorkshire ICB has issued a contract award notice for the "Pro-Active Frailty Healthcare Service - North East Lincolnshire". This contract, classified under geriatric services (CPV code 85121252), aims to provide proactive care and support for frail patients within a Primary Care Network by conducting Comprehensive Geriatric Assessments and developing individual care plans. The procurement process was executed under the Health Care Services (Provider Selection Regime) Regulations 2023, bypassing the Public Contracts Regulations 2015.
The contract, valued at approximately £771,422, was awarded on 5 February 2025, with the service term running from 1 April 2025 to 31 March 2026, and an option to extend for an additional year. This tender presents significant growth opportunities for businesses specialising in geriatric healthcare services. Suited for qualified providers capable of delivering high-quality, innovative, and integrated healthcare solutions, it offers a chance to support local healthcare delivery and improve patient outcomes.
Small to medium-sized enterprises (SMEs) with experience in comprehensive assessments and frailty management would be well-positioned to compete. The existing provider, Freshney Pelham NEL PCN, was noted for meeting the selection and key criteria, emphasising the importance of demonstrating technical and professional ability, social value, and a collaborative approach in tender applications.
The service identifies frail patients from a registered population within a Primary Care Network (PCN) and undertakes a Comprehensive Geriatric Assessment (CGA), generally in the patients place of residence, developing a care plan for each patient and referring to appropriate services. Individuals with a clinical frailty score (CFS) 7+ (post assessment) will be offered annual reviews, including proactive identification of those who are likely in the last year of life. The aim of the service is to provide proactive care to support frail patients to remain as well as possible as well as documenting advanced decisions through ReSPECT forms. Regular MDTs will be held to discuss patients' needs and identify support required. NHS Humber and North Yorkshire ICB is intending to award a contract an existing provider following direct award process C of The Health Care Services (Provider Selection Regime) Regulations 2023. Approximate lifetime value of the contract is PS771,422 Contract term - 1 April 2025 to 31 March 2026 (contract term may be extended by 1 year)
