NHS Humber & North Yorkshire ICB has awarded a contract titled "Pro-active Frailty Healthcare Service - North East Lincolnshire" under the Health Care Services (Provider Selection Regime) Regulations 2023. The service targets geriatric care, focusing on patients within a Primary Care Network (PCN) requiring comprehensive geriatric assessments and the development of individual care plans. This contract has been awarded directly to an existing provider, Apollo PCN, via a direct award process C.
The contract term runs from 1 April 2025 to 31 March 2026, with the possibility of a one-year extension, and has an approximate lifetime value of £513,926. Key dates associated with the process include the contract award date on 5 February 2025. This tender presents significant opportunities for businesses specialising in healthcare services, particularly those experienced in geriatric care and proactive patient management.
The contract's focus on comprehensive assessments and regular reviews creates avenues for businesses with capabilities in medical care, patient assessments, and coordination of multi-disciplinary teams. By participating in such tenders, businesses can expand their services within the NHS framework, improve their market presence, and contribute to the well-being of the elderly population in North East Lincolnshire. Businesses with innovative solutions in healthcare sustainability, quality care, and reducing health inequalities would be well-positioned to compete for similar opportunities.
The service identifies frail patients from a registered population within a Primary Care Network (PCN) and undertakes a Comprehensive Geriatric Assessment (CGA), generally in the patients place of residence, developing a care plan for each patient and referring to appropriate services. Individuals with a clinical frailty score (CFS) 7+ (post assessment) will be offered annual reviews, including proactive identification of those who are likely in the last year of life. The aim of the service is to provide proactive care to support frail patients to remain as well as possible as well as documenting advanced decisions through ReSPECT forms. Regular MDTs will be held to discuss patients' needs and identify support required.
