The service will also offer longer term support through relevant community networks and resources of the provider or by contact with VCS groups and networks. The service will act as single point of access and triage for assisted discharge support for discharge. It will ensure Service users are settled in safely at home after a spell in hospital and support service users to become independent at home again.
The service should contribute to the reduction of hospital re-admissions.
The contract will be for an initial period of 2-years, with an option to extend for two further periods, each of 1-year duration, commencing 1 October 2025.
