1. Purpose of Notice Cardiff and Vale University Health Board (CAVUHB) is undertaking a pre-market engagement exercise to inform potential suppliers of its intention to procure a new social prescribing digital platform with full integration to EMIS Web. 2. Background CAVUHB currently uses the Elemental social prescribing platform across Primary, Community and Intermediate Care services. The Health Board is exploring alternative digital options that can enhance: Integration with EMIS Web Referral workflows for GPs and link workers Assessment and outcome tracking Patient self-referral functionality Reporting, analytics, and dashboards Any future platform must also support SNOMED CT coding to ensure standardised data capture and NHS-wide interoperability.
3. Scope of Interest CAVUHB is seeking information from suppliers capable of delivering a social prescribing platform that provides: Integration with EMIS Web, including two-way data exchange for referrals, assessments, outcomes, and reporting SNOMED CT-coded data capture for referrals, interventions, and outcomes Online/mobile self-referral functionality for patients Service directory management and analytical dashboards Implementation support, training, and ongoing technical assistance Suppliers are encouraged to provide: An overview of their platform and functionality Details of EMIS Web integration SNOMED CT implementation approach Indicative pricing model or cost ranges References from NHS or Primary Care Network deployments 4.
