This service will focus on providing trauma informed responses and basic support for individuals who have been assessed as high risk. Assertive Outreach is a persistent and purposeful approach used to engage with individuals who do not present to, or have difficulties engaging with housing, homelessness, or health and social care services. This approach is particularly effective for people who are isolated, alienated, unwell, or disabled, and who are unable or unwilling to attend services and keep appointments It is estimated that the contract would run from 1 September 2025 to 30 November 2028 with a one year potential extension. The estimated contract value would be 86605 GDP per year.. This notice is designed to test market interest and capability to deliver this service, it is not a call for competition. If there are sufficient capable providers interested in this opportunity an appropriate procurement activity will be undertaken at a later date.
