Hywel Dda University Health Board (HDUHB) have an opportunity to expand their provisions of Bereavement Services for the North and South Ceredigion Cluster Areas and are looking to commission a suitable provider to undertake this work for the County's population. The successful provider will need to be able to communicate and liaise with our population through the medium of Welsh and provide in-person home contacts to the hard-to-reach areas across the rural geography due to limited public transport options, and patients' inability to travel independently, therefore, this would be a critical factor for the service provision. The Requirement To deliver a bereavement and anticipatory grief service to adults aged over 18 years in order to support a patients bereavement recovery and adjust to life after loss via various mediums including but not limited to, telephone, virtually via a suitable online platform and in person. There is a key focus on the in person approach to this service therefore an outreach model is essential. Due to the rurality of the area and the patient demographic, the ability to travel to patients to deliver the service in their own homes is essential to mitigate any factors that would limit patients ability to engage with the service Ceredigion has a high number of Welsh Speakers therefore it is expected that the service would be provided bilingually. The successful provider must: - Deliver a person centred bereavement service for adults aged 18 years and over throughout the County of Ceredigion - Deliver the service bilingually in Welsh and English - Offer an outreach service model to ensure that those unable to travel have the same equity of access - Work collaboratively with other public and third sector organisations to improve access to bereavement care for all, helping to safeguard primary care, by reducing the number of client visits to GPs - Support HDUHB strategy on health prevention and community resilience. - Promote Partnership working - Improve Mental Health & Wellbeing
