Summary, notice description and lot information
NHS Humber & North Yorkshire ICB has awarded a contract for the "Pro-Active Frailty Healthcare Service - North East Lincolnshire" under the Health Care Services (Provider Selection Regime) Regulations 2023. The contract, focusing on geriatric services, involves identifying frail patients within a Primary Care Network and providing Comprehensive Geriatric Assessments (CGA), care plans, and annual reviews for patients with clinical frailty scores of 7 or more. This service aims to support frail patients to remain well and document advanced decisions.
The contract was awarded on 5 February 2025 and will run from 1 April 2025 to 31 March 2026, with a possible one-year extension. The estimated contract value is £985,134. This tender offers substantial opportunities for businesses in the healthcare sector that specialise in geriatric care and community health services.
Businesses adept at delivering personalised care plans, conducting thorough geriatric assessments, and collaborating with multidisciplinary teams could find this contract advantageous. Given the contract’s requirement for proactive care and comprehensive patient assessments, companies with experience in managing frailty and end-of-life care would be particularly well-suited to this opportunity. Additionally, organisations capable of addressing health inequalities and enhancing access to care services are encouraged to participate.
The service identifies frail patients from a registered population within a Primary Care Network (PCN) and undertakes a Comprehensive Geriatric Assessment (CGA), generally in the patients place of residence, developing a care plan for each patient and referring to appropriate services. Individuals with a clinical frailty score (CFS) 7+ (post assessment) will be offered annual reviews, including proactive identification of those who are likely in the last year of life. The aim of the service is to provide proactive care to support frail patients to remain as well as possible as well as documenting advanced decisions through ReSPECT forms. Regular MDTs will be held to discuss patients' needs and identify support required. NHS Humber and North Yorkshire ICB is intending to award a contract an existing provider following direct award process C of The Health Care Services (Provider Selection Regime) Regulations 2023. Approximate lifetime value of the contract is PS985,134 Contract term - 1 April 2025 to 31 March 2026 (contract term may be extended by 1 year)
