Referrals for these services come from the acute wards via the Accommodation Gateway, which is a Leeds and York Partnership Foundation Trust (LYPFT) managed function. These services form an integral and essential part of the hospital discharge pathway in Leeds, and are an important resource used by the acute wards to ensure move-on and flow out of hospital
