CQC registered first time: how Breathe Care Services got ready before they applied

Breathe Care Services Limited was formed in August 2025. On 10 March 2026 CQC registered it as a home care agency, at the first attempt. This is the order the work happened in, what was built while the DBS checks ran, and how two people prepared for two very different roles.

Collins from Care Sync Experts on Breathe Care Services: onboarding, the application pack, sending the application within two weeks of the DBS checks, preparing the Registered Manager and Nominated Individual, and what the director said after the interview.

How do you pass CQC registration first time?

Have everything CQC needs in place before you apply, and send it all together. For a home care agency that means enhanced DBS checks back, your office set up, staff recruited, complete provider and manager forms, and accurate supporting documents. CQC rejects incomplete applications. Then prepare your registered manager and nominated individual for assessment.

What went into Breathe’s application pack

We built Breathe’s supporting documents from the ground up, around their service model, their area and their people. Nothing generic and nothing copied from another provider.

Business plan

Tailored to Breathe’s service model and local market. CQC’s additional form for home care providers asks you to show “market research in your local area to evidence there is a local demand for your specific service”, so the plan and the form have to tell the same story.

Domiciliary care business plan guide

Statement of Purpose

Written to include everything required by Regulation 12 and Schedule 3 of the Care Quality Commission (Registration) Regulations 2009, and matched to the service type and service user bands on the application.

Policies and procedures

A complete set for a home care agency, from safeguarding to governance to medicines management, written for how Breathe will actually work.

Training plan

The one people forget: your office

Care happens in people’s homes, but CQC still needs details of your main office. We guided Breathe through the evidence for theirs before anything was sent.

Even if you provide care in people’s homes, we need information about your main office. This is where you’ll manage your service and keep records.

  • No PO box addresses
  • The address must be in England
  • CQC may refuse an application if the premises are not suitable, for example if records cannot be kept safe and secure

How we helped

Just under seven months, from new company to registered

Breathe Care Services supports older adults, younger adults, people living with dementia and people with physical disabilities in their own homes, from an office in Bromsgrove. Two dates in this story are public record. Everything between them is the work.

Public recordBreathe’s account of the work

Applied under the stricter rules

Breathe applied after 1 July 2025, when CQC began routinely rejecting home care applications that are not complete or accurate on arrival. A corrected application counts as new and goes to the back of the queue. Passing first time under that process is the point of this story.

  1. Public record

    12 August 2025

Breathe Care Services Limited is formed

Ten days later a company secretary is appointed: the person now listed as Nominated Individual on the CQC register. That matters, because CQC’s rules say a nominated individual must be employed as a director, manager or secretary of the organisation.

  2. Breathe’s account of the work

    Onboarding

    Understanding the service first

We sat down with Breathe, gathered everything we needed and got clear on what they wanted to build, where, and for whom, before a single document was written.

  3. Breathe’s account of the work

    While the DBS checks ran

The whole pack was built alongside

Enhanced DBS checks take time that neither you nor CQC controls. So the business plan, Statement of Purpose, policies, training plan and office evidence were written while they processed, not after.

  4. Breathe’s account of the work

Within two weeks of the DBS checks

    Application sent to CQC

Because everything else was ready, Breathe sent their application within two weeks of the certificates arriving. No scramble, and nothing missing.

  5. Breathe’s account of the work

    Assessment

Initial checks passed, then the interview

The application went through CQC’s initial checks and on to assessment. By then, the Registered Manager and the Nominated Individual had both been preparing one to one.

  6. Public record

    10 March 2026

    Registered by CQC, first time

CQC’s register shows the service registered on 10 March 2026, for personal care, run by Breathe Care Services Limited.

    CQC registration

That is not a promise about your timeline. CQC says its assessment “can take a few months”, and every application is different. What Breathe’s dates show is how much of the calendar sat in their own hands.

How long CQC registration takes, stage by stage

Build while the DBS checks run

For most new providers the longest wait before applying is the enhanced DBS checks, and CQC cannot process an application without them. Most people wait for the certificates, then start writing. Breathe did it the other way round.

CQC says it “must receive everything at the same time”. Breathe’s went in within two weeks of the DBS checks coming back.

CQC DBS checks: why the Update Service does not count

Getting the application in is only half the job

The Fit Person Interview is where it all comes together, and it is where a lot of applicants fall down. Not Breathe.

We ran one to one sessions with both the Registered Manager and the Nominated Individual, walking them through every area CQC explores. The aim was never to memorise answers. It was to understand them well enough to explain them in their own words.

Between sessions, they used our CQC Interview Preparation Portal to practise in their own time, go back over weaker areas and build confidence before the day.

Feedback from the Owner

“Every question that CQC asked in the interview was something we’d already covered.”
Millie, Breathe Care Services’ director.

She told us they were completely confident they had passed before the result arrived. The result proved them right.

Two people, two roles, two kinds of question

At Breathe, the Registered Manager and the Nominated Individual are different people. That is common in home care, and it means preparing two people for two different conversations with CQC.

Registered Manager

Runs the service day to day

  • CQC describes the role as being “in charge of the day-to-day running of regulated activities”, sharing legal responsibility with the provider.
  • For home care, CQC expects experience with complaints, mental capacity assessments, safeguarding concerns and medication errors.
  • Needs a current enhanced DBS countersigned by CQC and a full work history from age 16, with any gap over four weeks explained.

Nominated Individual

Supervises how the regulated activity is managed

  • CQC’s Regulation 6 makes the nominated individual “responsible for supervising the management of the regulated activity”.
  • Must be employed as a director, manager or secretary of the organisation, be of good character, and have the skills and experience for the role.
  • CQC must refuse registration if a provider cannot show it meets this regulation and will keep meeting it.

Registered Manager, Nominated Individual, or both?

Six situations every new home care agency has to be ready for. Who should be able to answer each one best? Pick an answer to see the reasoning, drawn from CQC’s own guidance.

0 / 6 answered

  1. Situation 1

How will you know the registered manager is running the service well?

  2. Situation 2

A care worker reports a medication error on a morning call. What happens next?

  3. Situation 3

A daughter complains that her mother’s call was 40 minutes late.

  4. Situation 4

How did you recruit your registered manager, and why are they right for this service?

  5. Situation 5

A care worker raises a safeguarding concern about a colleague.

  6. Situation 6

How will the service keep meeting the regulations as it grows?

How long will your CQC registration take?

Nobody can honestly promise you a date. What you can do is take charge of every part that is yours, which is exactly what Breathe did.

Frequently asked questions

CQC says its registration assessment is rigorous and can take a few months. Before that, the DBS checks can take from around 14 days to up to 60 working days, depending on which CQC page you read. Breathe Care Services was formed in August 2025 and registered in March 2026.
No. CQC says it cannot process your application without the required DBS checks, and that until you have your certificate you will not be able to submit. Use the waiting time to build your supporting documents.
An enhanced DBS check with the right barred list information. If you are not a registered healthcare professional, it must be countersigned by CQC. CQC does not accept certificates from the DBS Update Service, and rejects applications with DBS checks over 12 months old.
Yes. Even though care happens in people’s homes, CQC needs details of your main office, where you will manage the service and keep records. It cannot be a PO box, it must be in England, and CQC may refuse an application if the premises are not suitable.
Yes, and at Breathe Care Services they are. The Registered Manager runs the service day to day. The Nominated Individual supervises how it is managed and must be a director, manager or secretary of the organisation. CQC says a registered manager can also act as nominated individual, normally where the organisation is small.
CQC does not publish a fixed list of interview questions, but its guidance says it expects home care managers to have experience with complaints, mental capacity assessments, safeguarding concerns and medication errors, and to understand the Health and Social Care Act 2008, its regulations and the Mental Capacity Act 2005.
CQC’s current home care guidance says to send your application only when everything is ready to start providing services, including completed DBS checks, your premises set up and your staff recruited.
CQC rejects it and emails you to explain why. You can correct and resend it, but it counts as a new application and does not keep its place in the queue. For home care agencies this has applied since 1 July 2025.
No. CQC says you must complete the application yourself. We help you build the supporting documents, check the pack before it goes in, and prepare your Registered Manager and Nominated Individual for assessment.
By law, the information set out in Regulation 12 and Schedule 3 of the Care Quality Commission (Registration) Regulations 2009. The service type and details on your application form should match it.

Breathe completed and sent their application themselves, as CQC requires. Our job was to make sure that when they pressed send, every document was ready and every document said the same thing.
