Outreach Services and Day & Night Sitting Services

OUTREACH: the delivery of outreach services to enable adults receiving support to lead a full and active life, maximising their independence, promoting their health and wellbeing and providing support. The service will be flexible and person-centred, and designed to address the needs of people in receipt of support, and ensure outcomes of Support Plans are met. Outreach and support activities may include but are not limited to: accessing the local community to meet outcomes, support with personal care, support with everyday activities including shopping, advice on healthy eating choices; support to attend appointments; support to access paid and voluntary work opportunities; support with money management; support with mobility and transfers where applicable; support to eat and drink; support to take medication; support to attend sporting and leisure activities. SITTING: the delivery of day and night sitting services to enable carers of adults to have a break from their caring responsibility by providing support to adults in their own homes. The service will be flexible and person-centred, implementing effective positive risk management and design services which address the needs of people in receipt of support, and ensure outcomes of Support Plans are met. Sitting services may include but are not limited to support with: personal care; mobility and transfers; taking medication, eating and drinking; safe disposal of clinical waste; and encouraging adults to be more physically active in their own home, to maintain good health and wellbeing.
