The service will operate during weekday evenings and weekends, and will be accessible via referrals from NHS 111, Clinical Navigation Hub, Emergency Departments, and General Practice. The service is designed to manage a wide range of same-day clinical presentations, including childhood illness, infections, and other conditions suitable for ambulatory care. It aims to complete episodes of care wherever possible, reducing unnecessary onward referrals and supporting continuity for patients with complex or multimorbid conditions. This will be a newly redesigned service delivered as a 12-month initial phase commencing from April 2026 associated with the evolving local strategy for neighbourhood care. This enables evaluation of the model's effectiveness in improving access, integration, and patient outcomes across the local health system.
