Potential providers wishing to be admitted on to the approved provider list must complete the Enrolment Questionnaire ("EQ") and be able to demonstrate that they meet all of the minimum criteria listed. Any provider that is able to demonstrate that they meet these criteria will be admitted on to the approved provider list and will be able to carry out LARC services and recharge the Authority for the activity undertaken.
