The set price the Council will pay for a completed Health Check will be PS21.00. Providers using POCT will be required to meet consumable costs at their own expense. Providers using phlebotomy services will be required to meet these costs at their own expense.
Providers will require the community tool kit at an approximate cost of PS500 at their own expense. The service is available for AQP's who can deliver Health Checks face to face within Derbyshire to join at any time throughout the duration of the contract term.
