This Intention to Award Notice is issued in accordance with the Health Care Services (Provider Selection Regime) Regulations 2023. The Procurement relates to the provision of the Dementia Link Workers Service. NHS Shropshire Telford and Wrekin Integrated Care Board (NHS STW) currently holds a contract with Alzheimer's Society for this service and anticipates no change to the current service provision. The Alzheimer's Society provides the Dementia Link Workers (DLWs) service across NHS STW as part of an ongoing programme of transformation work led by the NHS STW Dementia Steering Group and forms part of the NHS STW Dementia Vision. The model aims to provide people living with dementia, their families and unpaid carers, with the support and resources they need in order to live their life to its fullest and as they choose to. Aspects of this work include: * Provision of peer support groups. * Create opportunities to meet other people with dementia and their partners and families * Connect to local services for practical and emotional support * Help to locate and access community families/groups and events. * Work with communities to become more dementia friendly, develop more peer support opportunities, support and enable access to services and activities and continue to run support groups. * Work with people with a dementia diagnosis: connect with their communities, access local support and services, connect with peer support opportunities, enable the living well plan to be put into action. NHS STW is satisfied that the requirements for the provision of health care services are not changing considerably, i.e., they don't meet the considerable change threshold as set out in Regulation 6(10). The intention is to award a contract to the Alzheimer's Society, the existing provider, via the Health Care Services (Provider Selection Regime) Regulations 2023, Regulation 9, Direct Award C. The contract period shall be for a period 12-months from 1st April 2025 to 31st March2026. The estimated contract value is PS285,237.00.
